Job Summary: Responsible for program eligibility determinations and re-determinations for Intellectual /Developmental Disability services in Douglas and Jefferson counties. Responsible for the completion of all initial and annual functional eligibility assessments for the Intellectual / Developmental Disability Waiver.  Responsible for completion of Level II Pre-Admission Screenings and Resident Reviews for individuals with IDD seeking nursing facility placement.  Provide information about service options and affiliated service providers in the CDDO area.  Provide information regarding community resources within the CDDO area.

QUALIFICATIONS: Bachelor’s degree in human services area and at least two years’ experience in the IDD service field. Also, must have a valid driver’s license, reliable vehicle, and driving record acceptable to Cottonwood’s insurance carrier.  Must be able to pass all required background checks and drug screen.

Such assistance includes direct involvement in providing exposure, advocacy, and training conducive to success in a variety of community settings.

QUALIFICATIONS:Must have a high school diploma or equivalent. A valid class C driver’s license is required, along with a driving record acceptable to Cottonwood’s insurance provider. Experience in working with persons with developmental disabilities preferred. Must have the physical ability to stand for long periods of time, lift and carry at least 25 lbs., endure some physical exertion (assisting with transfers, wet mopping etc.), and be able to evacuate consumers in the event of an emergency. Must be able to intervene in the event of a physical conflict and provide assistance with personal care. Must have basic computer skills, be able to access Cottonwood email, and complete data entry. Job Summary: The Health Support Specialist accompanies Consumers to both on and off-site/face-to-face and tele-health related appointments by transporting the Consumer to/from the appointment (when appropriate), providing and securing appointment-related paperwork to/from the provider and serving as a liaison between the Consumer and healthcare office personnel and provider during the appointment.  Responsibilities may include passing noon medications at Day Services and assisting in the group homes during pandemic related closures.

QUALIFICATIONS: Valid driver’s license and a driving record acceptable to Cottonwood’s insurance.  Good office skills. Excellent organizational and interpersonal skills.  Ability to foster positive, professional relationships with Consumers, Consumer’s family-members who may attend the appointment, Cottonwood staff, and medical office personnel and providers. Ability to problem-solve and work independently to meet the Consumer’s support needs while in the community.  Physically able to evacuate Consumers safely in case of an emergency and able to intervene in the event of a physical conflict.  Able to lift and carry 20 pounds and to assist Consumers with wheelchair transfers to/from a vehicle and within medical offices.  High School diploma/GED required.  Previous experience with individuals with intellectual or developmental disabilities is preferred. Job Summary: The Retirement Enrichment Services Coordinator supervises direct support staff.  Responsible for provision of program services, events, transportation, and coordinates social events. The coordinator is responsible for ensuring the effective provision of retirement services to help provide an environment where individuals with developmental disabilities enjoy their retirement years while maintaining their skills and abilities.

QUALIFICATIONS: A Bachelor’s degree and/or 2 years of work experience in the field of human services.
