Job Summary: The Employment Consultant I is responsible for provision of quality employment services to a caseload of individuals. These services may include but are not limited to job development, placement, orientation support, transportation training, on-site consulting, conflict resolution, task analysis, disability awareness training to employers, accommodation training, and follow-up with both the employee and employer. Responsibilities also include understanding and working with a variety of funding sources. The consultant must have flexibility in their schedule and be available whenever a consumer is working.

QUALIFICATIONS: Must have a high school diploma or equivalent; bachelor’s or advanced degree is preferred. Must have at least two years of customer service experience; experience in social services is preferred. Must possess good verbal and written communication skills. Must be able to work independently and be available for a flexible working schedule. Must have a valid driver's license and driving record acceptable to Cottonwood's insurance carrier. Must be able to pass all required background checks and drug screen.  Experience with Microsoft Access and Excel preferred.

Job Summary: This position combines clinical nursing functions with a community centered practice approach.  The RN works with a team to provide clinical nursing assessments and direct care to designated Cottonwood consumers and coordinates the delivery of care as prescribed by medical practitioners in the community.  This position requires weekly availability with a negotiated schedule and occasional availability to cover for the full time RN’s and LPN’s planned absences.

QUALIFICATIONS: Degree from an accredited School of Nursing.  Must have current State of Kansas Licensed Registered Nurse credentials with intent to retain license and obtain required continuing education.   A valid driver’s license and a driving record acceptable to Cottonwood’s insurance carrier is required.   Must have an ability to work with patient/consumers, medical practitioners, families, personnel, and other community agencies.  Basic operational computer literacy is required.  Must have an understanding and willingness to work in a non-medical model setting with persons with cognitive and physical disabilities and their lay staff.

Job Summary: The Health Support Nurse provides quality health assessment and care to designated Cottonwood consumers in accordance with Cottonwood policies and in compliance with regulatory standards when the on-site provision of care is in the best interest of the consumer and agency. This position also provides health and medication-related instruction to Cottonwood consumers and to Cottonwood unlicensed staff, as appropriate, for delegation of nursing tasks, plus coordinates health-related visits for designated Cottonwood consumers who receive health care services in the community. The position also shares on-call responsibilities for telephonic support to Cottonwood Residential staff on evenings and week-ends.  RN and LPN candidates will be considered.

QUALIFICATIONS: Degree/graduation from an accredited School of Nursing. Current State of Kansas Registered Nurse (RN) license or Licensed Practice Nurse (LPN) license. Must have strong collaboration, organizational, and communication (both oral and written) skills. Ability to establish and maintain effective working relationships with others; which includes consumers, families, co-workers, healthcare providers, and other community agencies. Must have an understanding and willingness to work in a non-medical model setting. Must have basic operational computer literacy. Must have a valid driver’s license and a driving record acceptable to Cottonwood’s insurance.  Must maintain applicable nursing licensure. Job Summary: The Work Services Supervisor assists the persons served in developing work skills and the appropriate behaviors necessary to attain and maintain independent employment. The WSS will provide productive Career Development activities for the persons served when work is not available.  The WSS is responsible for completing appropriate documentation regarding the payroll and Person-Centered Support Plan for each person served.

Such assistance includes direct involvement in providing exposure, advocacy, and training conducive to success in a variety of community settings.

QUALIFICATIONS:Must have a high school diploma or equivalent. A valid class C driver’s license is required, along with a driving record acceptable to Cottonwood’s insurance provider. Experience in working with persons with developmental disabilities preferred. Must have the physical ability to stand for long periods of time, lift and carry at least 25 lbs., endure some physical exertion (assisting with transfers, wet mopping etc.), and be able to evacuate consumers in the event of an emergency. Must be able to intervene in the event of a physical conflict and provide assistance with personal care. Must have basic computer skills, be able to access Cottonwood email, and complete data entry.
