Current Support Services Job Opening

DEPARTMENT: Support Services             JOB TITLE: Support Services Coordinator/Case Manager
 
JOB SUMMARY:  Coordinates and monitors the quality of services and resources offered to persons served, ensuring the delivery of adequate and appropriate services, according to SRS, CARF and HCBS/MR Medicaid waiver standards/guidelines.

QUALIFICATIONS:  A Bachelor's degree and a minimum of at least six months experience in the field of services to individuals with mental retardation or developmental disabilities.  Special consideration will be given to individuals  that have extensive relevant experience in the field of services to individuals with mental retardation or developmental disabilities.  Noted experience may be substituted for any portion of a Bachelor's degree at a rate of six months of experience for each semester.   Must have a valid drivers license and a driving record acceptable to Cottonwood's insurance carrier.   Must be able to participate in specified targeted case management training, and registration as outlined by the CDDO & SRS.

SUPERVISION RECEIVED:  Director of Support Services.

JOB RESPONSIBILITIES:
*1.        Coordinate the development of the Person Centered Support Plan Process for each consumer on caseload.

 *2.       Ensure that consumer rights are respected at all times and informed consent is obtained, as needed.

 *3.       Coordinate the acquisition of community resources, as determined/recommended by the team. 

 *4.       Maintain quality of case records for those persons on caseload and keep all information, as defined by SRS, CARF, Ability One, and HCBS/MR Medicaid waiver standards/guidelines.
 
*5.        Assist consumers in obtaining appropriate financial assistance and benefits.

*6.        Serve as primary agency contact for guardians, parents, and families for issues regarding the consumer.

*7.        Assist and coordinate crisis intervention, and seek  professional service, as needed.

 *8.       Assist in seeking appropriate health and medical services.

 *9.       Assist with the Admissions and Discharge process of consumers on caseload.

*10.      Complete BASIS as outlined by SRS.

*11.     Develop, review, and update consumer plan of care.

*12.      Present positive, fair, and accurate image of the organization to consumers, the public and community representatives.
 
*13.     Collaborate and problem solve with other team members in a professional manner.  Promote team participation and negotiation and follow through with assigned responsibilities.

*14.     Communicate and document accurately, professionally and appropriately.

*15.      Document and submit billable services for all consumers on caseload, in a timely manner.

*16.     Work within prescribed schedule as approved by supervisor and complete responsibilities in a timely and efficient manner.

*17.      Serve on inter-departmental committees as assigned.

 18.       Perform other duties as assigned by supervisor.

*19.     Complete all training requirements for the position as outlined by SRS and the CDDO.

DEPARTMENT:  Work Services                      JOB TITLE:  Special Needs Supervisor

JOB SUMMARY:  The Special Needs Supervisor assists the workers in developing work skills and the appropriate behaviors necessary in attaining and maintaining independent employment. The SNS also has responsibility to assure that each worker has an active Person Centered Plan. The SNS is responsible for completing appropriate documentation regarding worker’s payroll and their PCP.

QUALIFICATIONS:  Must have high school diploma or equivalent. Experience in working with people with developmental disabilities is preferred. Must have ability to provide physical interaction as trained in managing behavioral issues.  Must have a valid drivers license and a driving record acceptable to Cottonwood’s insurance carrier.  A flexible attitude is essential.

THIS POSITION IS: Non-Exempt

JOB RESPONSIBILITIES:
*1.        Protect and promote the rights, dignity, health and safety of persons served.

*2.        Assure agency that the Cottonwood site enhances the independence, self-sufficiency and productivity of the persons served.

*3.        Assure agency that the Cottonwood site maximizes the functioning of the persons served.

*4.        Assure agency that SRS, CARF and agency policies, procedures and regulations are in compliance and implemented within areas supervised.

*5.        Assure agency of consumer satisfaction involving individuals served. Goal:  85% average on Consumer Satisfaction Survey, implemented semiannually.

*6.        Assure agency that each participant has a current ISP which meets the standards of the agency and CARF. Document any deficiencies.

*7.        Establish and assure compliance of production procedures within the work areas.

*8.        Assure quality and quantity control of work produced in the work areas.

*9.        Enhance the economic independence of individuals being served.

*10.      Assure that time sheets, incident reports, personnel action forms, worker ISP objectives, worker time cards and monthly contact notes are kept and turned in on time.

*11.      Model appropriate work, community, and interpersonal behaviors at all times.

*12.      Attend to the personal needs of individuals served; including help with meals and toileting hygiene as necessary.

*13.      Attend scheduled meetings, serve on agency committees, monitor break room and attend
             in-service training, as required.

*14.     Establish and maintain various records and schedules.
 15.     May be assigned van route responsibilities.                      
            
*16.      Be punctual and keep absenteeism to a minimum.

*17.      Transport and supervise consumers in a safe manner to and from various community
             settings.

*18.      Maintain positive professional working relationship with consumers, co-workers, supervisors, staff members, parents, and community representatives.  Present positive, fair, and accurate image of the organization to the public.

*19.      Collaborate and problem solve with other interdepartmental staff in a professional manner. Promote team participation and negotiation and follow through with assigned responsibilities.

*20.      Communicate accurately, professionally and appropriately.

*21.      Work within prescribed schedule and complete responsibilities in a timely manner.

*22.      Attend in-services as required to maintain competency in position.

*23.      Assure compliance with Cottonwood’s Policies and Procedures.

24.     Have the ability to lift and carry 40lbs, and provide physical interaction as trained in managing behavioral issues.

25.    May  be assigned the responsibilities of doing worker  assessment for Light Material

            Handling.

DEPARTMENT:  Work Services                 JOB TITLE:  Data Assurance Manager                                                 
JOB SUMMARY: This position is concerned with helping to assure the Agency that the data collected and time studies used in determining consumer pay and in figuring bids is accurate and in compliance with Department of Labor standards and regulations.  The Data Assurance Manager performs time studies on sub-contract work, hourly assessments and on jobs in the community and assists in preparing bid information. This position will also manage consumer work crews developed to provide consumer workers expanded work options.
QUALIFICATIONS: Production related work experience, knowledge of FLSA section 14© as it relates to time studies, proven experience with accurately maintaining data systems. Must be detail oriented, have computer and spreadsheet experience and excellent math skills. Must be well organized, self directed and motivated. Preferred knowledge of ISO Quality systems or auditing other quality systems.  Preferred experience working with developmentally disabled individuals.

JOB RESPONSIBILITIES:
*1. Performs and documents time study information. Prepares and files bid information.  Assure Agency that all contracts are bid properly, accurately and in compliance with Department of Labor standards and regulations.  Work cooperatively with other departments to perform time studies.
2.  Help with implementing hourly rated assessments.
*3 Establish, bid, and manage potential community work sites including the supervision of workers.       
*4.  Assist with maintaining job codes for new jobs, existing jobs and changes in jobs.  
5.  Serve as back-up with documenting, entering, updating, printing and distributing reports as needed.

  1. Vacation and sick leave hours used and hours available - monthly.     
  2. Earnings summaries - When requested.  These include; gross pay summary, consumer average hourly wage, average hourly wage per contract, gross & productive total wages, and material handling hourly rates.
  3. Cost analysis of current customer’s jobs.
  4. Consumer alphabetical list, numerical worker list and staff number list.
  5. Listing of job codes.                                                             
  6. Earnings history on individual workers as requested.

*6.  Assist with the survey of area businesses annually to arrive at prevailing industrial wage to be used in determining base hourly and piece rates.                                                 
7.  Assist with the Development/changing/updating forms and reports as needed.                       
8.  Assist with the “setting up”, “jigging” and “Quality Assurance” of new jobs.
*9.  Achieve competency in ISO quality system auditing to perform quality management system audits as lead auditor.
10.  May be involved with 1 on 1 training with consumers.
11.  May be asked to serve on various Agency Committees.
*12.  Assure compliance with Cottonwood policies and procedures.  Exemplify and promote safety, proper behavior and productive work habits in all phases of job and interactions with workers, staff, customers and the public.                                
*13. Be punctual and keep absenteeism to a minimum.
