Health Support Services

Health Support Specialist (Job Posting #2017-21)

Job Summary: Serve as a resource to the agency for health coordination related issues. Accompany consumers to scheduled appointments—medical, dental, etc.  Act as liaison regarding consumer’s medical history while at the appointment and thereafter with the consumer’s health assessment and recommendations.  Assist with the coordination of health-related appointments.  The Health Support Specialist will perform duties of the other Health Support department support staff when they are absent.

Qualifications: Must have a valid driver’s license and a driving record acceptable to Cottonwood’s insurance.  Must have good office skills that include telephone communication, filing, record keeping, computer literacy, fax, copier, organizational and interpersonal skills with efficiency and accuracy.  Ability to work with patient/consumers, families, personnel, and other agencies required. Must have ability to work independently with little supervision.  Must be physically able to evacuate consumers safely in case of emergency and intervene in the event of a physical conflict.  Must be able to lift and carry 20 pounds.  Must be able to assist with wheelchair transfers to and from a vehicle as well as to and from other chairs and medical equipment as required.  Must possess the ability to manage multiple tasks concurrently.  Must participate in training as designated by the manager.

Supervisor (Job Posting #2017-24)

Job Summary:  Qualifications: To be considered for this position, applicants must have a high school diploma or GED, a valid Kansas driver’s license, and be eligible for coverage on Cottonwoods auto insurance. Applicants must also be able to assist with physical transfers of people who use wheelchairs, be able to do basic data entry, and basic computer skills. All qualified applicants will need to pass the required back ground checks, drug test and have a good work history. Supervisors provide support to persons with developmental disabilities in community, leisure settings, paid work, and structured curriculum based activities. Responsibilities include but are not limited to, providing meaningful and fulfilling learning activities for participants on a daily basis, personal care, hand over hand assistance while working and participating in various settings, transporting to and from community events.
